1) Sexual abuse by a KIN or another regulated health care professional 2) A KIN or other regulated health professional has been terminated for reasons of professional misconduct, incompetence or incapacity 3) A KIN voluntarily resigns, relinquishes or restricts his or her practice and there is reasonable belief it is related to their conduct 4) Ended some type of employment or partnership due to professional misconduct, incompetence or incapacity 5) They have reason to believe that a KIN is incompetent of incapacitated
