Fixed exam difficulty for multi-choice

Selecting an exam difficulty now correctly overrides any settings specified by the user. e.g. if user had setting set to 4 maximum multi-choice count, but chose hard exam - this would still show 4. It should and now does show 8 multiple choices in this scenario. This fix is too consuming to apply to v3. V3 will exhibit bugged previous behaviour.
